   Access to Health Record

The practice is registered with the Information Commissioner's Office, which controls how we use our data.  All staff undergo annual training in confidentiality issues and Data Protection.  Under the Data Protection Act 1998 people have the rights to see any files about them, including their health records held by a GP, hospital, optician or dentist.  Access can only be denied if there are compelling reasons.

For information about fees payable please email  Either we can send copies out to you (a fee is payable for this), or you can come into the surgery to view the records by mutually convenient appointment. 

The Practice is fully computerised. (Registered under the Data Protection Act 1984).

Access to Health Record of Someone who has Died

Under the Access to Health Records Act 1990, only specific people are allowed to apply for access to the health records of someone who has died, for example the legal Personal Representative or any person who may have a claim resulting from the person's death.  Application for GP records must be made in writing to Ealing PCT.  You may have to pay a fee for this information.


  • Did you know GPs are no longer able to countersign your passport or driving licence application form? Opticians, dentists, civil servants, religious ministers, bank and building society officials and many other professionals can also do this for you.
  • Did you know you don’t need a sick note if you are off work for less than seven days? You can self-certify – contact your personnel office at work for details.
