   Joining The Practice


TO REGISTER at our surgery, you must live within our practice boundary.

Named GP

All patients, including children, will be allocated a named accountable GP who is responsible for the coordination of your overall care at the practice. This does not prevent you from seeing any GP in the practice.

Please note that you will be informed of your named GP at the point of registration.


NEW PATIENTS - Please read the 'How to register' document.

1. Do you live within our Catchment area? If yes please continue to read on. If not, you will have to find a GP near to where you live. you can do this online at

2. Download and print off the registration form(GMS1) and a questionaire form, which can be found below, or collect from reception.(Links to the forms are at the bottom of the page) Fill these out in BLACK INK & IN CAPITAL LETTERS;  you must fill in each section as this can cause delay at the desk when you arrive into the surgery.

3. Bring the forms in to reception with your proof of ID (Passport, birth certificate ) and proof of address (utility bill, council tax must be dated within the last 2 months).  Mobile phone bills will not be accepted for Proof of Identity or Proof of AddressChildren 0- 16yrs - Parents must provide the Red Book. If this is in another language this must be translated to English. This includes any immunisations given abroad. You will need your NHS number, if you do not have it you will need to contact your last surgery.  If you do not have proper proof you will be unable to register without it.

4. Once your forms have been checked, you will be asked to to do a mini health check, which includes blood pressure, weight and to provide a urine sample.

New patients should come to register with us betweem 10 - 5:00pm (and 10 - 12:00 on Weds). Please note we are closed on Tuesdays between 1pm-2pm.

You will need to be able to tell us your previous UK address and postcode, AND that of your previous GP.


You will need to complete one for each member of your family if you are registering more than one person.


Disabled Patient Facilities

Disabled patient facilities are available at this practice.  We have a wheelchair ramp at the front and back of the building.  All clinical consulting rooms are on the ground floor..
